Amdiglurax, also known as ALTO-100 or NSI-189 (originally developed by NeuroStem Inc.), is a small-molecule compound under clinical investigation. It has been described in research literature as a stimulant of hippocampal neurogenesis and an indirect modulator of brain-derived neurotrophic factor (BDNF). Development has focused primarily on neuropsychiatric indications, including major depressive disorder (MDD), bipolar depression, and post-traumatic stress disorder (PTSD). Additional research interest has explored its potential relevance to cognitive impairment and neurodegenerative conditions.
The precise mechanism of action of amdiglurax has not been fully established. Preclinical research suggests it may indirectly enhance BDNF signaling and influence neuroplasticity and neurogenesis within the hippocampus. In animal models, dose-dependent increases in hippocampal volume have been observed at higher exposure levels; however, similar structural changes were not demonstrated in a human clinical study. Amdiglurax is considered a first-in-class compound.
As of July 2024, amdiglurax remains in phase 2 clinical trials for MDD, bipolar depression, and PTSD, with a phase 3 trial for MDD under planning. Clinical findings to date have shown mixed results regarding efficacy for depressive symptoms, though some studies have reported improvements in cognitive and memory-related measures. The compound is currently being developed by Alto Neuroscience and was previously developed by Neuralstem, Inc. Outside of clinical research, it has also appeared in online markets marketed as a nootropic or antidepressant.
